Job Description
Milwaukee Health Services, Inc. (MHSI) is a Federally Qualified Community Health Center (FQHC) that operates from two (2) sites: The Isaac Coggs Heritage Health Connection at 8200 W. Silver Spring Drive and the MLK Heritage Health Center at 2555 N. Martin Luther King Drive.MHSI offers a range of primary health care services including Medical, Dental, Behavioral, Women’s Health, Benefit Determination, and Pharmacy. FQHCs service everyone regardless of income, or third party coverage, and seek to provide high quality care in accessible locations and at convenient times. Our particular sites primarily target the residents of Milwaukee’s north side, but service patients countywide.FQHCs are unique in their delivery of care, patient population and provision of services. FQHCs seek to provide quality care in an underserved area to the uninsured populations that would otherwise not have access to care.
 
  • Develop a comprehensive social history and psychosocial assessment that includes the client’s problems and strengths and preferences, including cultural, religious, and ethnic background and implications or the care plan.
  • Help clients (in their social, racial, ethnic, and cultural context) cope with the immediate effects of their current issues.
  • Orient clients to the Clinic, its services, and its services limitation and patient rights.
  • Empower clients and encourages clients’ choice in matters affecting them, demonstrating receptive attitudes and sensitivity to choices based on racial, cultural, and within heritage and sexual orientation.
  • Provide linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs.
  • Assist clients with financial questions and make appropriate referrals.
  • Develop and implement program policies and procedures.
  • Help clients cope with losses.
  • Consult with staff regarding psychosocial needs that encompass social, ethnic, religious, and cultural diversity of individual clients.
  • Advocate on a case, policy and program level.
  • Participate in policy decision making in a way that encompasses an understanding of the issues and implications relevant to religious, ethnic, a cultural diversity as it affects client’s life and care.
  • Participate in continuing education for certification/license and career upgrade.
  • Perform other job related duties, as may be assigned.

POSITION REQUIREMENTS

  • Education:           Completion of BSW or MSW degree through an accredited program meeting requirements for Wisconsin Certification.
  • Experience:         Experience in provision of an eclectic problem-solving approach to clients seeking short-term interventions and problem resolution. Familiarity with issues of poverty, community, family violence, HIV, human sexuality, AODA, criminal justice systems, chronic or terminal illness and pain, parenting skills, and gangs, are desirable.
  • Expertise:            Ability to provide culturally competent benefits services, to a variety of clients in a direct responsive manner. Willingness to confront traditional barriers in respect of benefits application for clients. Strong interactive "team" skills within a primary medical care setting. Excellent interpersonal skills and empathy towards patients. Able to develop teaching and counseling sessions, as needed. Able to adapt to changing situations and circumstances. Willing and able to assume and delegate the needs of clinic patients, as a member of the health care team.
  • Language:           Comprehend and use basic language either written or spoken to communicate information and ideals.
  • Hours of Work:    May vary based on Organizational need.
  • Travel:                 May vary based on Organizational need.
