OVERVIEW:

The Salvation Army Housing Programs were developed in order to provide both Emergency Shelter and Transitional Housing for homeless families transitioning from homelessness to independence. This position is responsible for providing case management services. The position also includes a role in Program Participant assessment, intake, and facilitation of program meetings. To handle case management related emergencies for the Housing Department as needed, the employee is required to live on site. POSITION REQUIRES the Residential Coordinator to live on site.

DUTIES AND RESPONSIBILITIES:

  • Conduct phone screens and interviews with potential program participants for all housing programs
  • Develop, implement case plans, and provides consistent individual case management in accordance with TSA and program standards
  • Provides individual case management for the emergency shelter and transitional housing
  • Maintain and update all program files, referrals, and case management notes
  • Communicate with and encourages program participants to maintain compliance with program requirements, as well as advancement towards reaching their case plan goals
  • Ensures program compliance and integrity for program participants
  • Ensures program participant confidentiality in accordance with the law and with TSA policy
  • Maintains and inputs required data into the Social Services and HMIS databases
  • Maintains a working "wait list" for potential program participants for the emergency shelter and transitional housing programs
  • Enforces program policies and disciplinary process, including disciplinary write ups for policy violations and terminations with approval of Director
  • Performs unit inspections to ensure program compliance in the areas of safety and sanitation for the emergency shelter and for transitional housing as needed
  • Facilitates Emergency shelter house meetings and assists program assistant with program workshops and transitional housing house meetings as needed
  • Participates in and assists with Housing Committee meetings
  • Organizes and assists with program events for program participants, including Family Nights and holiday parties as requested and/or needed
  • Collaborates with community agencies and attends community social service agency meetings as needed
  • Assists with the collection, recording, and reporting of program statistics on a monthly basis
  • Reports to Housing Manager, Director, and Corps Officers with regular program updates
  • Performs urine analysis tests on program participants as needed
  • Contacts program participants who have transitioned out of the program for 9-12 months for follow up services and assists with Alumni programs and events
  • Assists Housing Manager with scheduling and supervising volunteers and interns
  • Performs program duties in the absence of the Program Assistant or Housing Manager as appropriate
  • Is available on call for emergency situations related to program participant case management issues
  • Is on-call for TSA Housing program emergencies one weekend each month
  • Provides transportation to program participants in TSA vehicles as needed
  • Other related duties as required

EDUCATION AND EXPERIENCE:

  • High school diploma or equivalent (required)
  • CA Driver's License (required)
  • Bachelor's Degree in Social Sciences or the equivalent of education and experience
  • Experience with Social Services agencies and service providers within the community

  • Ability to lift up to 25 lbs. (usually file boxes)
