Overall Function: 

Facilitate all aspects of Comprehensive Community Services (CCS) service planning to include, but not limited to transporting consumers as needed, locate and make initial contact with referred individual, build rapport with individual, by using a strength based, trauma informed, recovery oriented, and culturally sensitive approach, develop an Individualized Recovery Plan within 30 days of the individual’s CCS application based on the assessed needs, authorize psychosocial rehabilitation services (mental health and substance abuse), develop measurable goals and frequency data to measure progress toward desired outcome, etc. 

 

Facilitate Individualized Recovery Plan reviews and update based on the needs of the individual or at least every six months.  
  1. Facilitate activities that ensure individuals receive assessment services, service planning, service delivery, and supportive activities/services in an appropriate and timely manner. 


  1. Ensure the Individualized Recovery Plan and service delivery for each individual is coordinated, monitored, and designed to support the individual in a manner that supports the individual achieve the highest possible level of independent functioning. 

  1. Support individuals in self-advocacy and assisting the individual obtain necessary services including, but not limited to medical, dental, legal, financial, and housing services. 

  1. Follow legal, organizational and contractual requirements, laws and policies (i.e.; County contract). 

  1. Prepare, complete and submit required items by due dates and required timelines. 

  1. Establish and maintain good communication, collaboration, and cooperation with all stakeholders. 

  1. Implement and/or practice the duties and responsibilities of a mandated reporter of suspected abuse under current policies and procedures, as required. 

  1. Attend all meetings and professional development activities as directed. 

  1. Coordinate intakes. 

  1. Perform other duties as assigned. 

 

Knowledge, Skills, and Abilities 

  • Thorough knowledge of federal, state and local laws, code and regulations concerning operations of program/services associated with chronic mental illness. 

  • Knowledge of protective payee responsibilities. 

  • Knowledge of chronic mental illness, dual diagnosis and treatment methods. 

  • Knowledge of methods for working with adults with mental illness diagnoses and developmental disabilities. 

  • Ability to maintain effective working relationships with department/community agencies/organizations providing resources/services. 

  • Ability to maintain confidentiality. 

  • Ability to communicate effectively with clients, staff, service providers and volunteers. 

  • Ability to develop, organize and implement variety of programs. 

  • Ability to understand and carry out instructions. 

  • Ability to meet and work with the public. 

  • Ability to work independently as well as cooperatively with co-workers. 

  • Attend trainings as required. 

  • Must have a valid driver’s license. 
  • Must have a reliable insure vehicle. 


Education

Bachelor's degree is required, Master's degree preferred.
