This member of the multidisciplinary team has the responsibility to carry out rehabilitation, support functions, and assist in the recovery process; provides substance abuse services, educational support, and consultation to families; provides crisis intervention services under clinical supervision of staff with professional degrees.
Responsibilities:
Be a member of the individual treatment team for an assigned number of consumers.
Participate in treatment planning and implementation; recovery support services; physical health and dental health monitoring; education and employment assistance; social development and functioning; activities of daily living (e.g., self care, living situation, nutrition, money management); and family structure and relationships.
Consult with community agencies and families to maintain coordination in the recovery process.
Document consumer progress to maintain a permanent record of consumer activity, according to established methods, policies, and procedures.
Participate in organization meetings, recovery planning, and review meetings.
Participate in providing treatment and substance use services.
Must have a reliable insured vehicle
Support
Provide practical help and support, advocacy, coordination, side-by-side individual support, problem solving, direct assistance, training, and supervision to help consumers obtain the necessities of daily living, including medical and dental health care; legal and advocacy services; financial support such as entitlements (e.g., SSI, SSDI, and Veterans Benefits), or housing subsidies (e.g., HUD Section 8); money-management services (e.g., payee ships); and transportation.
Education, Experience, and Knowledge Required
The Rehabilitation Specialist must have a Bachelor’s degree in the field of behavioral sciences, and have experience working with adults with severe and persistent mental illnesses, or individuals with similar human-services needs. May also have related training (e.g., certified occupational therapy assistant, home health care aide), or work experience (e.g., teaching) and life experience. Must have a strong commitment to the right and ability of each person with severe and persistent mental illnesses to live in normal community residencies; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.
