Job Summary:

This pharmacist will support population health strategies through outreach to patients for medication related quality measures. The population health pharmacist will be responsible for:
*Providing patient outreach to improve medication related quality measures through medication reconciliation, medication adherence support, and drug therapy optimization.
*Using motivational interviewing to assist patients in better adherence to their medications.
*Conducting population-level assessments and interventions to identify and address medication-related problems, gaps in care, and opportunities for improvement.
*Using data analytics and informatics to monitor and evaluate the impact of population health interventions on clinical, humanistic, and economic outcomes.
*Educating and empowering patients, caregivers, and health care providers on medication-related topics, such as disease prevention, health promotion, self-management, and shared decision making.
*Collaborate with population health pharmacy liaison, medication therapy management pharmacists, care management and clinic providers and staff to support patient care.

Job Description:

Supports providers and patients in assuring optimal patient medication therapy use by patients as part of an interdisciplinary care team. Practices in accordance with all applicable laws and regulations, following legal standards in all areas including, but not limited to, work rules, controlled substance requirements, state and federal regulations, and HIPAA regulations.

Principle Responsibilities

  • Patient medication optimization and education through individual patient assessment.
  • Identifies, prevents and resolves drug therapy problems.
  • Conducts medication histories to identify, prevent, and resolve drug therapy problems.
  • Analyzes patient medication regimens to reduce therapeutic duplications.
  • Counseling and Consultation.
  • Counsels patients on their medication regimen.
  • Provides upon request, consultation to clinic physicians, providers and other team members to assist in the individual care of patients.
  • Collaboration with Interdisciplinary Care Team.
  • Collaborates with physicians, providers, nurses, and other health professionals to better meet patients' needs while improving health care quality.
  • Participates in pharmacy department Medication Therapy Management collaboration team.
  • Provider Medication Education.
  • Presents on a specific therapeutic topic to discuss prevalent or problematic issues related to prescribing.
  • Medication Management and Distribution
  • Complies with all laws and regulations pertaining to the management and distribution of medications.
  • Follows legal standards in all areas including, but not limited to, work rules, controlled substance requirements, state and federal regulations, and HIPAA regulations.
  • Other duties as assigned.


Required Qualifications

  • Bachelor's degree in Pharmacy OR PharmD from an accredited school of pharmacy
  • 0 to 2 years of identified pharmacy experience


Preferred Qualifications

  • Completion of an accredited residency program
  • 2 to 5 years ambulatory care pharmacy experience including conducting assessment of patient medications, providing patient education, and performing care planning and consultation services


Licenses/Certifications

  • Licensed Pharmacist - MN Board of Pharmacy required upon hire if working in MN
  • Licensed Pharmacist - WI Dept of Safety & Professional Services required upon hire if working in WI
  • Pharmacist Board Certification preferred
  • Valid Driver's License required upon hire - MN and WI residents must obtain a valid driver's license in their state of residence within 60 days of employment
