Job Summary:
The Clinical Program Manager provides administrative leadership to a specialized Heart and Vascular Clinical Program. In collaboration with physician and clinical leadership, the Clinical Program Manager is responsible for developing and coordinating comprehensive and integrated disease specific program pathways, processes and quality metrics across the Froedtert Health enterprise. The incumbent ensures that cardiovascular care is evidence based; and coordinated across the care continuum and enterprise to decrease care fragmentation. He/she works with service line leadership to implement programs, and supports delivery of leading quality, cost-effective, and efficient care. The Clinical Program Manager provides training and development to employees and acts as a consultant to sites and departments facilitating an interdisciplinary approach to achieve positive outcomes through physician-led, patient centered care. The incumbent is an excellent communicator who is able to bring disparate groups to consensus in order to move care initiatives forward across multiple, enterprise-wide constituencies. He/she also ensures lasting organizational change and adherence to the programs through metric measurement and reporting outcomes at the enterprise, service line, division, and site levels. Other duties as assigned.

EXPERIENCE DESCRIPTION:

5-7 yrs. of experience in cardiac or vascular specialty area including program development experience is required.

EDUCATION DESCRIPTION:

Knowledge of nursing theory, practice and management to oversee planning and implementation and evaluation of patient care within more than one geographic unit is required. Bachelor's degree and Master's degree is required. Either the Bachelor's or Masters degree must be nursing. If Master's degree is not in nursing, graduate degree must be in associated field (e.g. Management, Organizational Leadership, Education, Business). 15 plus years of proven success in process/performance improvement with Six Sigma Belt/LEAN training may be considered in lieu of a Master's Degree.

LICENSURE DESCRIPTION:

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).. American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 3 months of hire. American Heart Association (AHA) Advanced Cardiac Life Support (ACLS) is required within 9 months of hire. CNS is highly preferred.
