MUST: 3+years of medical office experience Experience as a medical assistant and medical office support role
Comfortable working in an administrative support role Strong experience answering phones and working in a high volume environment Providing support to providers Handling care coordination Experience requesting medical records Strong customer service experience Experience using Microsoft Office Suite Strong medical terminology knowledge
DUTIES: The Duties of the Care Coordinator includes but is not limited to the following: Complete pre-visit planning for upcoming patient appointments Obtain medical records from other sites for care coordination Assist with care coordination and communication providers Obtain recent Emergency Room notes and/ or hospital discharge documentation Assist with developing systems and processes to ensure coordination of care Assist care team in implementing preventive measures, chronic care, and disease management Assist in the monitoring of care to ensure that care is coordinated between specialists Obtain immunization records via Qvera Document and update patient information in EMR Assist the care team to ensure patient compliance with the care plan
