Case Manager

Reports to: Case Management Manager


Position Summary:

Provides case management services to patients through case planning, case
management for secondary care services, information, and referral. Performs professional case
management activities involving both routine and difficult cases. Serves as a liaison between Volunteer Licensed Health Care Providers, Community Provider Partners, and other community agencies in meeting healthcare needs of the community.

Core Responsibilities:

Case Management: communicates with patients via phone or email to inform them of referral process; obtain relevant health information while documenting on patient's electronic intake form.

Provides case management and follows up on all secondary care referrals.

Performs case management for more difficult cases; confers with superiors on difficult assignments.

Screens patients for eligibility for medical and social services within the PCAN Network and other social service and medical providers in the tri-county area.

Assesses treatment needs: develops, monitors, and evaluates treatment plans and progress of service recipients.

Ensures that referrals are processed and connected timely to the appropriate provider specialist or Community Provider Partner.

Tracks patient's case and follows up with Community Provider Partner's coordinators to verify patient's appointment attendance, observing and evaluating treatment, and advocating for patient's need of services.

Manages patient's records by reviewing case notes and logging care progress.

Closes cases: preparing final treatment records, in-kind value amounts, discharge plans and treatment results.

Requests medical records from Community Provider Partner's offices.

Assists Health Information Data Analyst with quarterly reporting through review and analysis of patients' cases.

Regularly attends community-based case management meetings and serve as an agency resource for medical assistance options and programs that serve low-income patients.

Maintains comprehensive and up-to-date Community Resource Materials for Staff & Volunteers and consistently access the 211 system.

Responds to email and telephone correspondence in a prompt and timely manner.
Other duties as assigned.
