The Addiction Care Coordinator plays an important role in Meriter Hospital's efforts to help patients understand and access the treatment options for substance use. Their primary focus is to meet with patients throughout the hospital to help connect them to services after treatment. As a program that supports harm reduction, the Care Coordinator will be empowered to "meet the patient where they are at" to ensure they are connected to whatever treatment they feel ready for.
The day to day of the Care Coordinator will vary greatly depending on the volume of patients and their needs. The ideal candidate is someone that thrives with variety, is able to self-direct with organizing their day, and is capable of communicating with the many different stakeholders involved in a patients care. Depending on the situation they may need to gather information from the staff on the hospital unit, family members of patients, and advocate with community resources to find the right treatment options.
The Addiction Care Coordinator will work closely with a team of two other Addiction Care Coordinators and an Addiction Medicine Doctor. They will receive regular clinical supervision and support from the team at the NewStart Outpatient clinic.
