We are hiring a Family Medicine Physician at our Lisbon Avenue Health Center in Milwaukee, WI. This is a full-time position that does not require working late nights, weekends, or holidays. Compensation dependent upon experience and local market.

More than 100 employees support our agency's mission to improve the health and quality of life of the community by providing culturally competent services that address identified needs. We provide high quality, comprehensive care to roughly 16,000 patients annually'a majority of who are low-income. No one is ever turned away from our clinics based on insurance status or ability to pay. We value a patient-centered philosophy and provide range of services that include family medicine, internal medicine, women's health, urgent care, dental, radiology and integrated behavioral health. What will you do?
The Family Medicine or Internal Medicine Physician provides primary health care for individuals in the Primary Care Practice commensurate with training in accordance with general policies and standards of practice, as established by Progressive Community Health Centers. The Physician delivers care appropriate for an out-patient, primary care community health center with a diverse multicultural patient population in an urban location. As a member of our provider team, the Physician elicits detailed patient histories and conducts complete physician examinations and reaches diagnosis and orders appropriate laboratory tests and referrals, etc., as well as counsels patients and families on health and disease.

Responsibilities
  1. Provides all physician care generally provided at an ambulatory care facility, on an outpatient basis. Responsible for the delivery of care, treatment, and referrals. Arranges for referral resources for those medical specialties not provided by the corporation for patients of the clinic.
  2. Evaluates patients, based on medical training, to determine the medical condition of the patient through the performance of physical examinations, thereby, establishing a diagnosis and proposed treatment plan.
  3. Maintains high standards of medical practice and participates in the training development of the medical team. Assists in medical education of the clinic patients and clinic staff.
  4. Facilitates the patients transition within and between health care settings including referring and transferring patients for specialty care and provides follow-up care and discharges patients as necessary.
  5. Fulfills 40 hours of Continuing Education Credits within each twelve (12) month period. Attends professional conventions and seminars, participates in professional societies, and does all things reasonable, necessary and desirable to maintain and improve his/her professional skills
  6. Counsels patient regarding medical and other problems, which might affect the patient's health and well-being and documents each office procedure, prescription, consultation and patient contact in the patient's chart and on the clinic's encounter forms, within a 24-hour period.
  7. Performs such clinical procedures for the patient's benefit which are within the score and skill of practitioner and which are economically feasible.
  8. Prepares forms, letters, and examinations on behalf of the patients for claims of disability, workers compensation, injuries and illnesses, etc.
  9. Provides direction and leadership to the clinic's medical support staff, with regard to the administering of a patient's care.
  10. Performs other job-related duties, as may be assigned.

Requirements
  1. Graduate from an accredited school of Medicine and Residency Program required.
  2. Licensed in the State of Wisconsin
  3. Board eligibility in Family or Internal Medicine, required. Board certification preferred.
  4. Previous experience in a Family or Internal Medicine setting, preferred.
