Description

Purpose:

The Medical Services Coordinator facilitates medical and other supports to people with Intellectual Developmental Disabilities focused on personal outcome and independence.

Responsibilities

  • Reports directly to the Assistant Director
  • Schedule, facilitate and follow up on medical appointments and orders per DHS requirements
  • Collaborate with RN trainer prior to medical appointments and provide written report regarding appointment outcome
  • Ensure appointment documentation is completed, signed by residents, guardians and filed in a timely manner
  • Participate in medication administration and training of ADSP staff Per RN trainer, in accordance with agency policy and procedures and DHS Rule 115 and 116
  • Manages Medical closets, First Aid, PRN and MAR documentation supply
  • Communicate professionally with medical providers, community member, families, individuals served and team
  • Transcribe new medication orders per RN trainer and deliver medications as needed
  • Facilitate supports during emergent medical needs
  • Facilitate training to Individual Supports Providers per RN trainer and DHS OJT standards
  • Work collaboratively with RN Trainer, QIDPs, Supervisors, outside providers and families
  • Facilitate transportation supports to and from appointments (agency vehicle)
  • Follow daily responsibilities as assigned
  • Respond to crisis effectively
  • Attend and contribute to team meetings
  • Complete and maintain DSP required trainings and certificates
  • Provide Direct Supports as needed
  • Provide on-call supports for the department as part of a rotation

Requirements

  • Must be 21 years of age
  • High School Diploma or GED equivalent required
  • Satisfactory results on the Test of Adult Basic Education (TABE) or Test of Functional Health Literacy in Adults (STOFHLA)
  • Valid Illinois Driver's license and clean driving record
  • Completion of the DSP curriculum as specified by DHS
  • Completion of CPR and 1st Aid
  • Ability to lift to 50 lbs.
  • CNA certification preferred
  • 2 years' experience in the Human Services Field preferred
  • Familiarity with Judaism and Jewish Orthodox community preferred

  • Compensation: the minimum compensation for this position is $20.51 (Final offer will be based on experience).
