Job Description

POSITION SUMMARY:

BMC offers various services to individuals with substance use disorders, including care for pediatric primary care for children and adolescents whose parents are in recovery through the SOFAR program (Supporting Our Families through Addiction and Recovery).

The Family Navigator, based in the SOFAR clinic, will coordinate care and assist patients and their families in navigating the various systems they encounter during their parenting and recovery journey. This role will be part of a multidisciplinary team consisting of pediatricians, social workers, and navigators based in a pediatric outpatient hospital setting. The Family Navigator will work closely with their team to ensure the family's needs are being met by providing care coordination for the child and the parents within BMC and elsewhere. They will closely collaborate with team members from BMC's other adult addiction treatment services. The Family Navigator will meet with families during clinic sessions, be available on the family resource line, and collaborate internally and externally with providers, schools, and other agencies. They will provide administrative support by performing general clerical work, updating patient lists, and assisting with program data management.

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Responding and triaging calls received on the family resource line
  • Coordinating care for children and their parents/caregivers
  • Connecting parents to appropriate medical and mental healthcare providers, internally and externally
  • Assisting families when they are in the clinic by responding to their needs, scheduling future appointments, and coordinating any additional care needed
  • Ensuring the families have all necessary information to follow through with appointments (location of appointment, contact information, forms to bring to appointment)
  • Making referrals to outside agencies, including but not limited to EI and CBHI services
  • Follow up with families to assess their experience of various services and supporting clients with advocating when services are not meeting their needs
  • Assisting with forms and letters for the family's healthcare, school, social service, and transportation needs.
  • Following up on missed appointments
  • Collaborating closely with other SOFAR team members as needs arise
  • Working closely with other programs at BMC (RESPECT, OBAT, CATALYST, and others) to ensure quality of care
  • Developing and maintaining strong relationships with the community and community resources to ensure patients have access.
  • Working closely with patient's schools by attending IEP meetings when appropriate and coordinate with teachers/special education services
  • Managing EMR patient list and other program data

JOB REQUIREMENTS

EDUCATION:

  • Minimum Bachelor's Degree in Social Work, Psychology, Human Services, or related field.
  • Training related to substance use disorder treatment required


CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

  • N/A


EXPERIENCE:

  • Experience (paid or volunteer) working with families of various backgrounds required
  • Familiarity navigating community resources and systems required
  • Experience working with patients with substance use disorders required
  • Informal or formal training pertaining to working with patients with substance use disorders required
  • Experience working with Electronic Health Records preferred
  • Experience working on a multidisciplinary team preferred


KNOWLEDGE AND SKILLS:

  • Knowledge of substance use disorders and/or mental health required
  • Medical knowledge, assessment abilities, communication, and people skills required
  • Strong English communication (oral and written), interpersonal, organizational, and record-keeping skills required
  • Ability to handle multiple tasks and responsibilities at the same time effectively required
  • Comfortable working in a fast-paced environment preferred
  • Bilingual or multi-lingual skills appropriate to the patient population served preferred
  • Ability to work in a multi-cultural, diverse, decentralized environment, as a member of a health care team and comfortable working with multi-cultural, low-income families preferred
  • Knowledge of software applications such as Microsoft Excel and Word preferred Ability to interpret and record data and understand basic medical terminology preferred
