Job Description:

The Behavioral Health Provider assesses patients and/or patient's family, caregivers, and/or legal representatives and arranges for needed interventions. Plans interventions to help patients cope with social, emotional, economic, and environmental problems and provides short term treatment planning for mental, emotional, and behavioral disorders, conditions, and addictions.

Scope

The Behavioral Health Provider will work at our Green Valley Ranch Clinic and Northfield Family Medicine. This position provides care appropriate to the established plan of care in the assigned patient population.

The Certified Social Worker (CSW) utilizes clinical expertise to perform psychosocial assessment and develop/ implement a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. This position works collaboratively with patients, patient's family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patient's needs, broker services, coordinate care and transition plans, and advocate for patients. The CSW performs these duties under the direct supervision of an LCSW.

Job Essentials

  • Provides comprehensive intake examinations and behavioral health assessments and determines accurate DSM-5/ICD-10 diagnoses based on clinical information.
  • Provides behavioral health services to individuals, couples, families, and groups, using a variety of treatment approaches and tools.
  • Makes appropriate referrals for patients needing services outside the scope of the immediacy of primary care.
  • Maintains complete and timely clinical records on all clients.
  • Provides consultation to other staff members as requested and participates in staffing of cases.
  • Provides consultation and education services to individuals, agencies, institutions and groups as assigned.
  • Participates in staff training and attends staff meetings.
  • Responsible for the review of client files to assess dangerousness and violence evaluation of family violence referrals.
  • Responsible for making referrals to a variety of social services and medical resources as needed.


Minimum Qualifications

  • Master's degree in Counseling or Social Work. Education must be obtained from an accredited institution. Degree will be verified.
  • Current BLS certification from the American Heart Association
  • Two (2) years clinical experience in related field is required.

  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
  • May be expected to stand in a stationary position for an extended period of time.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$27.02 - $48.00
