Job Description

Covid Vaccination is a requirement for employment at Jefferson for employees working at Jeffersonrsquo;s clinical entities or at the University. If you are not currently vaccinated you will be required to receive the vaccination prior to hire date if you are offered employment, unless you request and receive an approved medical or religious exemption from Jefferson.

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.

Minimum Education and Experience Requirements:

Education:

Bachelorrsquo;s in Social Work (BSW) required. Masterrsquo;s degree (MSW) required and two years experience in a hospital setting, preferred

AND

Experience:

Ability to integrate knowledge of crisis intervention and counseling regarding a wide variety of psycho-social difficulties, selecting from a wide variety of therapeutic modalities. Must have experience with and knowledge of the aging process, systems theory and discharge planning theory. Experience and comfort using principles of trauma-informed care, and providing trauma-focused therapy.

PRIMARY FUNCTION: Provides comprehensive assessment of and intervention with patients followed by the Jefferson Addiction Multidisciplinary Service in order to: (1) address the full range of psychosocial needs presented by patients who have required Psychiatry/Addiction Medicine (JAMS) consultation, particularly those who present with substance use disorder diagnoses. Interfaces with all necessary personnel, payer groups, agencies, and physicians to arrange appropriate disposition for the patient.

ESSENTIAL FUNCTIONS:

  • Collaborates with all members of the healthcare team, patient and/or family to develop and implement a patient centered plan of care.
  • Interacts with co-workers, visitors, and other staff consistent with Jeffersonrsquo;s mission, vision, and values.
  • Advocates for evidence-based and effective care of substance use disorders, psychiatric disorders, and comorbid medical conditions.
  • Performs and documents a comprehensive psychosocial assessment for each identified patient, including a mental status examination and diagnostic summary for patients who present with substance use disorder and/or behavioral health issues.
  • Obtains, interprets, communicates, and documents necessary information regarding patients and families, including social history information to the JAMS amp; healthcare team.
  • Provides counseling and psychoeducation to patient/family as appropriate. Practice demonstrates understanding in the following elements of the counseling process:
    • Liaising effectively between patients/families and involved teams including JAMS, primary healthcare teams, and Peer Specialists (CRS).
    • Providing short-term individual therapy to patients as clinically indicated
    • Facilitating telemedicine group therapy to select patient groups as clinically indicated and when consented to by patients
    • Providing trauma-informed care
  • Contributes to development and implementation of behavioral plans/safety plans for hospitalized patients with SUD.
  • Collaborates with the care team(s) to facilitate the discharge plan as it relates to the patientrsquo;s level of care.
    • Performs or contributes to standardized level of care assessment to establish consensus regarding post-acute plan with patient and care team.
    • Collaborates with CRS and others to develop relapse prevention plans at discharge, whether planned or unplanned (ldquo;AMArdquo;)
    • Arranges intake into outpatient MOUD programs and assists with warm handoff
    • Facilitates connection to appropriate outpatient resources including Intensive Outpatient Care, Partial Hospitalization, Case Management Services, Outpatient psychiatric/psychotherapeutic/co-occurring services as clinically indicated
    • Assists medical/surgical social workers in liaising with complex inpatient psychiatric/co-occurring/substance use treatment placements as required


      This position would be starting July 1st, 2024.
