Location: US:WI:MENOMONEE FALLS at our FROEDTERT MEN FALLS HOSPITAL facility.

Shift: Shift 1

Monday - Friday: 8:00-16:30, every 5th weekend on call.

Job Summary:

Serves as a resource for the Froedtert &MCW system hospitals and healthcare team in evaluating patients' medical needs and eligibility for an Inpatient Rehabilitation (IPR) level of care. Engages in care coordination rounds with the acute care teams to expedite consults for IPR admissions and/or other PM&R needs. Reviews medical records, conducts pre-admission screens, and communicates timely with PM&R consult physicians, IPR Medical Director/admitting physicians and Admissions Coordinator to verify patient eligibility for IPR and to coordinate safe and timely transfers to the inpatient rehabilitation unit. Responsible for developing and nurturing professional relationships with case managers, physicians and clinicians on acute care hospitals units. Educates staff, patients and families on the goals of medical rehabilitation and fosters strong relationships to build business through an increase of referrals and admissions. Tracks referral activity, admissions decisions and efficiency metrics and reports results to the Rehabilitation Leadership team.

EXPERIENCE DESCRIPTION:

A minimum of 3 years of clinical experience in nursing or rehabilitation therapy is required. Clinical experience in Inpatient Rehabilitation and familiarity with hospital admissions procedures preferred. Previous marketing/sales experience preferred.

EDUCATION DESCRIPTION:

Bachelor's Degree in Nursing or a related field required.

SPECIAL SKILLS DESCRIPTION:

Strong clinical assessment skills, critical thinking skills and judgment. Highly organized, with the ability to manage time and prioritize work flow. Ability to be self-directed and work autonomously. Strong customer service and interpersonal skills necessary to interact with all stakeholders, physicians, insurance representatives and referrers. Effective written and verbal skills. Computer skills including EMR navigation, data management and presentations, and report writing preferred.

LICENSURE REQUIRED:

Current state of Wisconsin licensure as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist required. American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent.
