Job Purpose:

The Service Facilitator is responsible for the coordination of services for youth in the Comprehensive Community services program of Washington County and to ensure needs of youth and family are met by providers to facilitate greater overall mental health and well-being.

Duties:

  • Performs comprehensive assessments and develops and implements client service plans.
  • Provides individual and family psychoeducation and skill development
  • Develops a network of community support and resources, connects clients with resources as appropriate or makes referrals as needed
  • Establishes and maintains working knowledge of WCHSD programs, resources and supports
  • Facilitates team meetings
  • Maintains client files and participates in quarterly review of files
  • Monitors outcome data to measure program performance

Qualifications:

  • Master's degree in Social Work or equivalent required.
  • 2 years' experience in child and family services
  • Ability to work within a multi-disciplinary team
  • Excellent written and oral communication skills
  • Exceptional organizational skills

Position Details & Extras:

  • Full time,primarily located in our West Bend office.
  • Onsite / Hybrid work is available with reliable broadband connection.
  • Fast paced, flexible work schedule includes some evenings and in-home hours
  • Moderate exposure to noise.
  • Subject to frequent interruptions with ability to manage multiple tasks simultaneously.
  • Demonstrates an understanding of the cultural differences among diverse groups and the need to adapt service provision to match these differences in respectful ways.
  • Public Service Loan Forgiveness (PSLF) - Wellpoint employees may be eligible for loan forgiveness under the Public Service Loan Forgiveness program.
  • Interaction with personnel, clients, family members, visitors, volunteers, interns, and vendors under all conditions and circumstances.
  • Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
