Position Summary:

As supervised and delegated by a physician, provides direct patient care in accordance with applicable scope
and standards of practice in procedural-based office.

ESSENTIAL FUNCTIONS
Collects and documents patient intake data. Maintains accurate and complete patient records.
Assists with catherterizations
Administers medications/injections
Provides patient education to both patient and family
Assists with patient procedures in procedural-based office

Minimum Qualifications:

Minimum Education
Graduated from LPN, LVN program

Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be
able to work in a stressful environment and take appropriate action.

Licenses:
Licensed Practical Nurse (LPN)

Certifications:
Basic Life Support Health Care Provider (BLS-HCP)

Minimum Work Experience
2 years Procedural experience (Preferred)
