JOB SUMMARY
The Speech/ Lang Pathologist at Scott Memorial Hospital is responsible to evaluates and treat patient populations for
swallowing, speech-language and therapy needs as requested by physicians.

Minimum education
Bachelor's degree Preferred X Required
Master's degree X Preferred Required
Doctoral degree Preferred Required
Required Licenses
[Indiana, United States] Speech-language Pathologist
Licensed Speech Therapist in the State of Indiana
Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to
work in a stressful environment and take appropriate action.
ESSENTIAL FUNCTIONS
Performs and documents initial and ongoing assessments of patient's condition.
Designs and implements a plan of care which is appropriate to problems identified and involves the patient and
family.
Educates and counsels patients and families regarding treatment plans and progress.
Plans and/or assists with patient discharge from therapy services.
Supervises activities of clinical fellows, students, and other support personnel.
Consults with the referring physician in any questionable situation relating to the evaluation and treatment of therapy
needs.
Provides patient follow-up programs as documented in the progress and discharge reports
Documentation of patient treatment, outcomes progress
Completion and submission of reports and/or other documentation in a timely manner and as required by State and
federal agencies.
Maintenance of patient treatment schedule consistent with needs of patient.
Provides treatment of patient according to the plan of care.
