Community Care is hiring a Registered Nurse - Home Care for our PACE Program. Registered Nurse - Home Care Responsibilities:
Monitors, delivers and documents routine nursing care in clinics or community settings including hospitals and residential facilities. Oversees cares and interventions delegated to Licensed Practical Nurses (LPNs), Medical
Assistants, Nursing Secretaries and Personal Care Workers (PCWs).
Performs nursing assessments as needed and participates in Nursing Home Care planning meetings.
Assists members in maintain optimum health; provides health education and counseling to members and caregivers facing chronic conditions and end of life issues.
Registered Nurse - Home Care Job Requirements:
Education: Associates in Nursing required. Bachelors in Nursing Preferred.
Experience: A minimum 1 year of personal, clinical/internship and/or employment experience providing care or services for a frail or elderly population. Excellent clinical and assessment skills. Working knowledge of principles of specialized treatment and training techniques related to adults with physical and/or developmental disabilities and dual diagnoses. Knowledge of multiple systems of care for elder adults and persons
with physical disabilities. Experience with Electronic Health Record. Preferred: Experience in a community-based or long term care setting involving the elderly, 2 years of community nursing.

Monday to Friday
8:00 am - 4:30 pm
