Reporting to the Supervisor Case Management, the Social Worker is responsible for coordinating the care of designated patients and families during a patient's hospital admission. The Social Worker coordinates the discharge plan from pre admission through discharge follow up. The Social Worker provides psychosocial assessment and updates that address the patient's evolving

$15-$20/hr; plus $1.30/hr 2nd shift differential rate

Observes patient in inpatient setting to ensure their comfort and safety. Communicates closely with the registered nurse to have a better understanding of the patient's needs, diagnosis, visual, auditory or language barriers, along with any other special considerations.
