Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Ryan White Housing Program Coordinator that is self-motivated, energetic, and a take charge individual.

Job Responsibilities:
  1. Meet with eligible Ryan White (RW) patients who walk-in or are referred and presenting with housing needs, to:
    1. Determine eligibility and assist with enrollment the RW Housing Program at Sixteenth Street.
    2. Assist with goal-planning related to housing needs such as obtaining money for security deposit.
    3. Assist with housing search that meets patient needs.
    4. Transport or meet with patients at potential housing units to verify ownership and conduct Housing Quality Inspections and determine if the unit meets the patient’s needs prior signing agreements with landlords.
    5. Write letters of support for rental applications.
    6. Review the lease with clients, so they understand their responsibilities related to rent payment, damages, termination, and occupancy terms.
    7. Accompany to lease signing as needed.
    8. Assist with moving arrangements including assistance to complete paperwork necessary to access and maintain other services related to housing (e.g., utilities).
    9. Assist patients to secure household goods and furniture.
  2. Provide crisis prevention and intervention services to eligible RW patients to secure maintaining housing including:
    1. Assistance with goal planning to transition to long-term, stable and independent living.
    2. Submit requests for payment for rent on behalf of eligible RW patients and make sure they are processed in time to prevent risks of eviction.
    3. Link RW eligible patients to support services such as Linkage to Care and others listed in PCN 16-02 from the HAB in order to prevent disruption in access to care.
    4. Provide personalized housing education pertaining to patient’s current and future living situation to help them further develop living skills such as budgeting, tenant rights and responsibilities, self-advocacy, good tenant, and neighbor behaviors amongst others.
    5. Assist to resolve conflicts between patients and landlords and if necessary, re-house patients in difficult circumstances.
    6. Conduct periodic assessments and follow ups to determine continuous eligibility to the RW Housing Program at Sixteenth Street including revision of income, expenses, and past due debt.
  3. In coordination with HIV Director:
    1. Develop, implement, revise, and follow policies and procedures to be compliant with RW Housing services guidance.
    2. Develop and maintain networking relations with key community partners to create a strong support system for people living with HIV.
    3. Develop and maintain strong relationships with landlords to facilitate client placement and to manage crises, resolve housing issues, respond to landlord concerns, etc.
    4. Monitor caseload and available resources.
    5. Track progress towards goals.
  4. Maintain detailed documentation of interactions with patients and all services provided.
  5. Serve as housing resource for HIV case managers throughout the Milwaukee area.

Qualifications:
  1. Bilingual (Spanish/English) fluency preferred.
  2. Associates or bachelor’s degree in Social Work, Human Services, or similar discipline or 2-year work experience in related field.
  3. Experience working with people from diverse ethnic, cultural, socioeconomic backgrounds.
  4. Knowledge on housing laws and regulations, preferred.
  5. Excellent organizational skills.
  6. Excellent interpersonal verbal and written communication skills.
  7. Knowledge of community resources and social service networks.
  8. Experience using MS Office, EMR and web-based programs.
  9. Valid Wisconsin driver’s license, use of an automobile and current automobile insurance.
