Job Description
We are seeking a PrEP Navigator that is self-motivated, energetic, and a take charge individual.
 
Job Responsibilities:
  1. Conduct PrEP education and linkage to access to client at significant risk for HIV> .
  2. Develop community education to high risk communities to engage in routine HIV/STI testing and increase uptake of pre-exposure prophylaxis and adherence.
  3. Assist client’s access to post exposure prophylaxis.
  4. Follows the Wisconsin State HIV/HCV and Syphilis Counseling, testing and referral and Rapid testing protocol.
  5. Assess clients for high-risk behavior by performing a behavioral risk assessment and referring them for appropriate risk and/or harm reduction programs, prevention interventions, HIV testing, prevention group sessions and syringe exchange.
  6. Provide direct individual or group level prevention intervention services to high-risk individuals as defined by the CDC or the State prevention interventions.
  7. Provide HIV/HCV and other STI testing and referrals for all clients requesting them.
  8. Provide risk and/or harm reduction education to clients, offer referral options and assist clients to link and connect to appropriate resources.
  9. Complete all state sponsored HIV testing, counseling and prevention trainings and develop competence in the use of the Evaluation Web data system.
  10. Complete required data entry in evaluation web and in SSCHC EHR when appropriate.
  11. Participate in all prevention initiatives sponsored by the Wisconsin AIDS/HIV Bureau and related trainings.
  12. Maintain a client file for each participant in the prevention interventions.
  13. Submit monthly reports on encounters and intervention goals to HIV Manager.
  14. Submit quarterly narrative reports on details of the prevention intervention progress to HIV Manager.
  15. Participate in the annual preparation of the State prevention grant applications.
  16. Attend monthly meetings of the HIV Department and the Prevention Committee.
  17. Assist in developing promotional materials to recruit clients.
  18. Assist in developing/securing culturally appropriate educational materials for clients and SSCHC patients.
  19. Participate in occasional weekend and evening educational events related to HIV prevention.
  20. Work closely with other members of the HIV department to provide comprehensive services for all clients and patients.
 
Qualifications: 
  1. Bachelor’s Degree in social work, human services desired, or equivalent work experience or 2 years case management or medical navigation support preferred.
  2. Ability to relate well to people from diverse ethnic and cultural backgrounds.
  3. Excellent organizational skills
  4. Excellent oral and written communication skills.
  5. Valid Wisconsin driver’s license, use of an automobile and current automobile insurance.
  6. Complete all trainings required by the WI AIDS/HIV Program within 6 months of hire.
