Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Community Lead Outreach Coordinator that is self-motivated, energetic, and a take charge individual.
 
 Job Responsibilities:
  1. Act as primary SSCHC contact person for the Milwaukee Health Department’s (MHD) Community Lead Outreach Program (LOP) and perform activities outlined by the program:
    1. Community Engagement
      1. Conduct outreach to community organizations that serve children and families (e.g. early childhood centers, day care centers, churches, schools, etc.) and provide education about Childhood Lead Blood Poisoning and Prevention. Outreach activities may include: presentations (in person or virtually), small group discussions, and tabling at events.
      2. Establish and maintain relationships with community organizations and prioritize outreach to promote services. Coordinate, schedule, and conduct outreach activities and coordinate other LOP staff to assist.
    2. Family services
      1. Provide in-home or virtual lead poisoning prevention education to patient and clients, including calling families referred by the MHD to the LOP, home assessment for lead risks, cleaning protocols, capillary (finger-stick) blood lead tests for children under age six who have been referred by the MHD.
    3. MHD Lead Safe Homes Kits Coordination
      1. Distribute Lead Safe Homes Kits to community members.
      2. Track inventory and coordinate replenishment of water filters, education materials, and the MHD’s Home Lead Kits.
      3. Submit reports on kit distribution to MHD.
  2. Accurately track and document outreach and home-visit activities and compile monthly reports of activities for the Milwaukee Health Department.
  3. Participate in meetings with the Manager, Lead Program Coordinator, City of Milwaukee, Social Development Commission, and other community organizations.
  4. Connect families directly to social services and community programs via a warm hand-off and provide property owner educational resources as appropriate.
  5. Assist with Lead Program responsibilities relative to home visit scheduling, visit documentation and organizing materials for home visit activities.
  6. Create and present presentations to community organizations and design educational materials for community outreach work.
  7. In partnership with the Lead Program Coordinator, coordinate door to door outreach to promote lead education in communities around SSCHC.
  8. Other duties as assigned by the Manager.
     
    Qualifications:
  1. High school diploma or GED.
  2. Experience in community-based agency, community health organization or related field.
  3. Ability to relate well to people of various ages and from diverse ethnic, cultural and socioeconomic backgrounds.
  4. Ability to work independently to build the program, recruit partners, and foster relationships within the community.
  5. Bilingual proficiency (Spanish/English required).  
  6. Excellent oral and written communication skills, including ability to speak before groups of people.
  7. Proficient in Microsoft Office (Word, PowerPoint, Excel).
  8. Attention to detail and organizational skills.
  9. Ability to work occasional evening and weekend hours.
  10. Valid Wisconsin driver’s license, use of reliable automobile and current auto insurance.
  11. Must obtain SSCHC’s internal certification for the administration of CLIA finger stick blood lead tests within first three months.
