Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Community Lead Outreach Worker that is self-motivated, energetic, and a take charge individual.
 Job Responsibilities:
  1. Act as primary SSCHC contact person for the Milwaukee Health Department’s Community Lead Outreach Program and perform activities outlined by the program:
    1. Community Outreach
      1. Conduct outreach to community organizations that serve children and families (e.g. early childhood centers, day care centers, churches, schools, etc.) and provide education about Childhood Lead Blood Poisoning and Prevention. Outreach activities may include: presentations (in person or virtually), small group discussions, and tabling at events or in lobbies, etc.
      2. Establish relationships with community organizations to promote services. Coordinate, schedule, and conduct outreach activities
    2. Family services
      1. Provide in-home or virtual lead poisoning prevention education to patient and clients, including: home assessment for lead risks, interim control, capillary (finger-stick) blood lead tests for children under age six, who have been referred by the Milwaukee Health Department
  2. Accurately track and document outreach and home-visit activities and provide information as needed to the Director
  3. Participate in meetings with the Manager, Lead Program Coordinator, City of Milwaukee, Social Development Commission and other community organizations.
  4. Provide referral information and support to families regarding social services and community programs and property owner education regarding lead-based paint and safety during renovation
  5. Assist with Lead Program responsibilities relative to home visit scheduling, visit documentation and organizing materials for home visit activities
  6. Other duties as assigned by the Manager or Lead Program Coordinator
  1. High school diploma or GED
  2. Experience in community-based agency, community health organization or related field
  3. Ability to relate well to people of various ages and from diverse ethnic, cultural  and socioeconomic backgrounds
  4. Bilingual proficiency (Spanish/English required)
  5. Excellent oral and written communication skills, including ability to speak before groups of people
  6. Proficient in Microsoft Office (Word, PowerPoint, Excel)
  7. Attention to detail and organizational skills
  8. Ability to work occasional evening and weekend hours
  9. Valid Wisconsin driver’s license, use of reliable automobile and current auto insurance
  10. Must obtain SSCHC’s internal certification for the administration of finger stick blood lead tests
