Job Description
We are seeking a full-time HIV Prevention Navigator that is self-motivated, energetic, and a take charge individual.
 
Job Responsibilities:
  1. PREVENTION NAVIGATION AND OUTREACH
    1. Coordinate and conduct outreach activities with the purpose of identifying individuals at high risk for HIV infection, increase education and awareness about prevention services and facilitate access to Counseling Testing and Referrals (CTR).
    2. Provide CTR services for HIV/Hepatitis C Virus (HCV) and other STIs for all clients during office visits or during outreach activities. Outreach activities may take place occur during evenings or weekends.
    3. Conduct Social Determinants of Health (SDOH) assessments with clients accessing CTR and offer navigation services to connect them to available resources within Sixteenth Street and other community agencies. Navigation services may also include the provision of education, translation/interpretation, and other support based on client needs to facilitate access to services.
    4. Assist clients testing positive for HIV, HCV or STIs to navigate systems to access medical care, treatment and other services including linkage to care, case management, and other resources in-house or at other agencies.
    5. Provide education on overdose prevention and participate in the distribution of Naloxone and assist with the Needle Exchange Program.
    6. Maintain files for each client participating in prevention interventions complying with State protocols and complete required data entry in the appropriate platforms.
    7. Complete all state sponsored Counseling Testing and Referrals (CTR) and prevention trainings and stay up to date on trends regarding HIV, HCV and STIs prevention.
    8. Under the coordination of the HIV Prevention Supervisor participate in strategic planning process, the preparation grant applications, and submission of progress reports for all the prevention interventions.
    9. Work closely with other members of the prevention team to successfully achieve the goals of the department while maintaining the organization of the Prevention Center to offer an inviting environment to our clients, this may include keeping supply inventories, procuring the cleanliness of office and work areas, and contributing with ideas for improvement of customer service experience.
 
  1. COMMUNITY ENGAGEMENT
    1. In coordination with the Prevention Supervisor and other Prevention team members:
      1. Connect with community partners, businesses, other stakeholders, and residents to develop partnerships and identify/create opportunities for collaborations, increase awareness and acceptance of HIV prevention strategies and related services including CTR, PrEP, syringe exchange, risk reduction and harm reduction.
      2. Connect with key staff at agencies providing community resources and get acquainted with their workflows to facilitate access for our clients.
      3. Monitor outcomes of referrals and assist in evaluating patterns of barriers that clients may face when attempting to access resources and work toward resolution as appropriate.
      4. Facilitate and/or participate in community events.
Qualifications: 
  1. Bachelor’s Degree in social work or human services desired, consideration will be made for a suitable combination of previous academic and professional experience working with diverse populations including individuals who identify as LGBTQ and populations at risk for HIV, we are willing to train the right candidate.
  2. Excellent oral and written communication skills. 
  3. Ability to relate well to people of various ages and from diverse ethnic and cultural backgrounds.
  4. Excellent organizational skills and ability to work independently and in a group setting.
  5. Proficiency with MS Office, experience with MS Access or similar database platforms.
  6. Valid Wisconsin driver’s license, use of an automobile and current automobile insurance.
