Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Family Educator that is self-motivated, energetic, and a take charge individual.
Responsibilities Include:
Provide education and supportive services to individuals and groups on various Social Determinants of Health (SDoH) including but not limited to parenting, safe housing, economic stability, access to community resources, and child safety.
Complete intake needs assessment and develop a plan of services for patients and non-patients requesting services.
Address and respond to complex group dynamics to promote a positive learning environment.
Identify indicators for children at risk for child abuse, neglect, or sexual abuse and respond accordingly to State of Wisconsin requirements for mandated reporters.
Be prepared to manage behaviors and issues that are identified in parenting settings to de-escalate stressful situations while maintaining the safety of the children.
Refer patients and non-patients to other programming offered through SSCHC as appropriate.
Promote use of services offered within SSCHC and throughout the community.
Enter, update, and maintain electronic documentation of all encounters, recommendations, and referrals made.
Provide supervision of children during parent provider appointments.
Participate as an active member of the Social Service Department including staff meetings, in-serve training and workshops, department problem solving exercises and providing support to co-workers.
Qualifications:
Two years of experience in early childhood program, educational setting, or social service agency with experience in the duties described above.
Ability to become a certified Community Health Worker within twelve months of hire.
Comfortability providing services in person and over virtual platforms.
Ongoing training in SDoH, trauma informed care and facilitation methods.
Talented working in individual and team settings.
Excellent oral and written communication skills.
Knowledge of social, educational and health services throughout the community.
