Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time HCV and Harm Reduction Navigator that is self-motivated, energetic, and a take charge individual.

Responsibilities:
  • Conduct Counseling Testing and Referrals (CTR) for HIV, Sexually Transmitted Infections (STI) and Hep C with people at high risk for infection who come to Sixteenth Street for services or at outreach settings.
  • Identify PWID and conduct harm reduction interventions, including education on safe syringe use/exchange, overdose prevention, HEP C testing and treatment.
  • Connect people presenting with HIV, STIs and/or Hep C infection to appropriate Sixteenth Street’s resources/ departments. 
  • Assist People who Inject Drugs (PWID) expressing interest to connect to substance use and AODA services at Sixteenth Street or other providers in the community as appropriate.
  • Screen PWID and others presenting with Hep C infection to identify barriers to care and offer referrals and assistance to connect to services and resources at Sixteenth Street or in the community to overcome needs related to Social Determinants of Health (SDOH).
  • Participate in all Sixteenth Street HIV, HEP C, and STI prevention projects including outreach activities.
  • Maintain encounter records, including HIV, HEP C and STI CTR documents, following confidentiality and HIPAA protocols and complete required data collection and encounter documentation in EvaluationWeb, Sixteenth Street’s EMR and other platforms as appropriate.
  • Participate in preparation of grant applications and reports required by funding sources.
Qualifications:
  1. Bilingual (Spanish/English) fluency preferred.
  2. Bachelor’s Degree in social work, human services preferred or 2-year work experience in case management or certified Community Health Worker. 
  3. Experience working with people from diverse ethnic, cultural, socioeconomic backgrounds.
  4. Excellent organizational skills.
  5. Excellent oral and written communication skills.
  6. Valid Wisconsin driver’s license, use of an automobile and current automobile insurance.
