Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Perinatal Care Coordinator that is self-motivated, energetic, and a take charge individual.

Responsibilities: Provide case management to all patients of Sixteenth Street Community Health Centers beginning at onset of prenatal care until 8 weeks postpartum, responsibilities include but are not limited to; performance of risk assessments at the beginning of each pregnancy and as necessary, refer patients to Sixteenth Street provider or community agencies/specialists as needed, health and breastfeeding education; assess prenatal patients’ knowledge and provide education on labor readiness and newborn care, educate patients on contraception options including natural family planning, comply with the State of Wisconsin requirements for Perinatal Care Coordination (PNCC) as outlined in the Sixteenth Street policies and procedures and the PNCC Handbook, maintain appropriate patient records for PNCC and OB Medical Home (OBMH) programs, conduct home visits for clinic patients as required by PNCC and OBMH guidelines and as requested by Sixteenth Street providers, conduct a minimum of one postpartum home visit to review patient self-care, newborn care, family planning methods, and provider instructions.  Visit may be conducted by phone or at one of the Sixteenth Street locations if patient desires, if medically indicated, or if patient resides outside of Milwaukee County, support breastfeeding success by performing prenatal and postpartum education by phone, clinic visit, or home visit, facilitate breastfeeding classes and support groups.
 
Qualifications:
  1. High School Diploma or GED
  2. Completion of lactation consultant training program, certification highly recommended
  3. Knowledge of Sixteenth Street Community Health Center service community
  4. Ability to relate well to people from diverse ethnic and cultural backgrounds
  5. Computer and typing skills
  6. Valid Wisconsin driver license, use of an automobile and current automobile insurance
