Works with the Director of Nursing, Wellness Nurse Manager, and Nurse Manager – Assisted Living in meeting organizational goals and objectives related to resident wellness and care. Serves as a liaison and advocate for residents and families, ensuring their psycho-social needs are identified and met, in accordance with the facility’s mission, resources, policies, procedures, and state and federal regulations. Develops and monitors plan to serve social and emotional needs of residents through custom services. In partnership with Life Enrichment Manager and Nurse Manager – Assisted Living, implements and coordinates therapeutic activities for residents in assisted living.
Minimum Requirements:
Bachelor’s degree in Social Work or Nursing; or Associate’s degree in Occupational Therapy, Physical Therapy, Nursing, or Applied Science or equivalent required.
Minimum two (2) years of work experience in geriatric social services, assisted living, or long term care
