Clinical Evaluation Coordinator (Admissions Coordinator)

Education Requirements: Degree in Physical Therapy Assistant, Occupational Therapy Assistant , LPN, Social Work.

Full-time 1st shift

Some responsibilities include:

§ Evaluates candidates for admission through review of medical information and makes admission decisions consistent with clinical guidelines

§ Responds to requests for general information and admission inquiries received by the Lutheran Home & Harwood Place

§ Triages inquiries by health need, timing and payer source.

§ Meets with unexpected guests that may inquire in person regarding admission

§ Schedules and conducts tours of the facility for clients, prospects, and/or family members

§ Provides information on the availability of specific services and accommodations to prospective residents and families

§ Obtains complete demographic, financial, and social information from prospective clients or family members.

§ Contributes to review and updating of clinical and financial admission guidelines based on patterns of inquiries, admissions and denials

Minimum Requirements

§ Degree LPN, Physical Therapy Assistant, Occupational Therapy Assistant, or Social Work. Must be licensed in Wisconsin.

Preferred medical and/or therapy background.

§ Experience reviewing clinical information

§ Excellent verbal and written communication skills

§ Excellent interpersonal and customer services skills

§ Ability to organize and prioritize workload

§ Attention to detail

§ Experience with and comfortable entering and managing electronic data

§ Proficient in Outlook, Microsoft Word and Excel
