Responsibilities:
Conducts Wellness Exams as assigned
Assist with triage and telephonic needs of patients
Assist in smooth workflow of clinic by:
Assisting on the clinical floor as needed
Assist with emergent and acute patients
Execute nursing interventions through collaborative work with Providers and support staff
Provide care coordination, develop treatment plans, and assist with the development of patient self-management goals for complex adult patients not covered by other programs or insurances
Connect patients with case management services provided through their coverages
Evaluate readiness and progress towards goals for complex patients as assigned and communicate such with Providers and support staff
Provide education, information, direction and support related care goals
Assist in maintenance/protection of patient privacy and medical record data collection
Participate in the Clinic mission to provide high quality healthcare
Perform specialized clinical procedures (Synagis injections, catheterizations, IV insertions, phlebotomy, Stress testing, NST’s) as needed
Proactively participate in staff development by mentoring, seeking to provide “on-the-spot” learning opportunities, creating in-servicing plans, delivering in-servicing plans
Comply with SSCHC Infection Control Plan including employee testing
Maintain additional assigned clinical projects (staff development, Coumadin clinic, other administrative duties) in a safe, orderly, and documented manner, with minimal supervision requirements
Qualifications:
Registered Nurse with current license and two years nursing experience; current basic life support certification (CPR)
Supervisory experience or demonstrated supervisory aptitude
Strong computer skills preferred
Experience in community health, public health or in a community agency
Ability to relate well to people from diverse ethnic and cultural backgrounds
