Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full time PrEP Navigator that is self-motivated, energetic, and a take charge individual.

Responsibilities include: assessing clients for high-risk behavior by performing a behavioral risk assessment and referring them for appropriate risk reduction programs, prevention interventions, HIV testing, prevention group sessions and syringe exchange, provide direct individual or group level prevention intervention services to high-risk individuals as defined by the CDC or the State prevention interventions, provide HIV testing, testing and referral for all clients requesting them, provide an annual “Prevention for Positives” session for each active HIV+ patient treated at SSCHC, complete all state sponsored HIV testing, counseling and prevention training sessions and develop competence in the use of the Evaluation Web data system, attend training on use of the CDC DEBIs (Diffusion of Effective Behavioral Interventions) and use them to facilitate effective prevention programs, participate in all prevention initiatives sponsored by the Wisconsin AIDS/HIV Bureau and related trainings, develop and facilitate prevention and support groups for high-risk individuals, maintain a client file for each participant in the prevention interventions and submit data to the WI AIDS/HIV Program using their report system. Submit monthly and annual reports, assist in developing promotional materials to recruit clients, Assist in developing/ securing culturally appropriate educational material for clients and SSCHC patients, participate in occasional weekends and evenings educational events related to HIV prevention, present educational sessions on HIV at schools, churches, and local agencies and health fairs.


Successful applicants will have a Bachelor’s Degree in social work or equivalent work experience, two years direct client experience in a social service or health agency, ability to relate well to people from diverse ethnic and cultural backgrounds, excellent oral and written communication skills, bilingual (Spanish/English) fluency required, valid Wisconsin driver’s license, use of an automobile and current automobile insurance, complete all training required by the WI AIDS/HIV Program within 6 months of hire.
