Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Social Services Patient Navigator that is self-motivated, energetic, and a take charge individual.
 
Responsibilities include: Complete Patient Navigator training within six (6) months of hire date, receive referrals of patients identifying needs during Universal Screening process, assess patients’ needs using motivational interviewing skills and assist them with referral process as needed, determine appropriate service level too meet the needs of patients in an effective and efficient way, educate patients about self-advocacy, show them how to research and access resources needed for when facing critical life situations, develop a service plan with patients presenting mid-level needs, document all patient encounters describing needs, service plan, progress notes, referrals and outcomes in E.H.R., schedule appointments for patients with high-level needs with a Social Worker taking into consideration patient self-determination, document intake assessments of patients with high-level needs as part of the referral process to Social Worker to facilitate seamless care coordination, follow up with patients, via phone call, to remind them of appointments in Social Services, refer patients presenting with mental health crisis to BH Open Access, monitor tasks and respond to provider’s communication in E.H.R., research, collect and maintain contact information of resources in the community, develop and cultivate relationship with resources in the community to facilitate smooth.

Successful applicants will have the following: 
  1. Human Services Associate degree preferred or 2 years of experience working in Social Services field.
  2. Motivational interviewing training and experience preferred.
  3. Excellent oral and written communication skills.
  4. Experience working with MS Office applications.
  5. Ability to relate well to and communicate well with people of various ages and from diverse ethnic and cultural backgrounds.
  6. Bilingual (Spanish/English) required.
