Job Description
Join our team who is committed to the delivery of the highest quality health care service.  We are seeking a full-time Lead Outreach Worker that is self-motivated, energetic, and a take charge individual.
                                                    
Responsibilities include:

Provide in-home lead poisoning prevention education to patient and clients, including: home assessment for lead risks; interim control, finger-stick blood lead tests for children under age six, door-to-door canvassing to homes in the community to identify young children who have not been tested for lead poisoning, Provide referral information and support to families regarding social services and community programs and property owner education regarding lead based paint and safety during renovation, Participate in environmental health community education presentations, e.g. health fairs, education to child care programs, and media, Assist with Lead program responsibilities relative to home visit scheduling, visit documentation and organizing materials for home visit activities, Accurate data entry and documentation related to home visit.

Qualifications:
  • High school diploma or equivalent
  • Experience in community- based agency community health organization or related field
  • Ability to relate well to people of various ages from diverse ethnic and cultural backgrounds
  • Bilingual communication skills required (Spanish/English)
  • Good oral and written communication skills in both languages
  • Experience in data entry using Excel, Access, or equivalent computer background
  • Ability to work occasional evening and weekend hours
  • Valid Wisconsin driver’s license with the use of an automobile and current auto insurance
  • Ability to obtain SSCHC’s internal certification for to the administration of finger stick blood lead tests
