Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time HIV Nurse Case Manager that is self-motivated, energetic, and a take charge individual.

Responsibilities include: providing and coordinating care and treatment for new and existing SSCHC patients in the HIV program, work collaboratively with the physicians, behavioral health providers, social worker/case manager, RN case manager, the prevention specialists and outreach specialists to assure the coordination of quality health care for the patients with HIV, order laboratory studies per HIV protocol and assign the new patient to a physician designated to treat HIV, refer patient to HIV social worker/case manager for social intake and assistance with related issues, following initial appointment with the physician, instruct patient as to MD’s assessment and care plan,  provide direct nursing services to patients, as directed by the physician, appropriately document all patient encounter/interactions, including phone contacts, in EMR as well as in the HIV Database, review patient’s Progress Notes in Electronic Medical Record (EMR) for care plan and orders. e.g. in-house or external referrals, labs, x-ray etc. Review tasks in EMR throughout the day for new orders, provide ongoing patient education regarding medication compliance, HIV transmission and prevention, HIV progression, safer sex/ safer IV drug use and confidentiality, make patient home visits to facilitate compliance with health care plan and medication adherence.

Successful applicants must have be a registered nurse with a current license in WI, two years nursing experience desired, experience in Community Health, Public Health or a community agency and direct experience to persons with HIV preferred but not required, Current BLS or ACLS certification, ability to relate to people from diverse ethnic and cultural backgrounds, valid Wisconsin driver’s license, use of an automobile and current automobile insurance,  bilingual (Spanish/English required.)
