Job responsibilities: Manage and oversee the care coordination programming at Sixteenth Street Community Health Centers including Chronic Care Health Education Program (CC), Perinatal Care Coordination Program (PNCC), Child Care Coordination Program (CCC), OB Medical Home (OBMH), Population Health, and any new programming that may be developed, develop programmatic goals and objectives, monitor outcomes, and implement internal audit procedures related to above listed programs, monitor the state of Wisconsin requirements for PNCC, CCC, and OBMH programs and adjust policies, procedures, and protocols as necessary, monitor the Accountable Care Organization (ACO) and HEDIS requirements that Sixteenth Street is responsible for reporting on and design the work of the Population Health Specialists accordingly, work in collaboration with the Sixteenth Street Grant Writers to identify appropriate funding opportunities for above listed programs. Collect and provide the Grant Writers with any reportable data that they require
Qualifications:
BSN degree in Nursing OR managerial experience in the healthcare field
Has a minimum of three years' supervisory experience
Excellent oral and written communication skills
Valid Wisconsin driver's license, use of an automobile and current automobile insurance
