Job Description

Join our team who is committed to the delivery of the highest quality health care service. We are currently seeking a part-time Certified Diabetes Educator that is self-motivated, energetic, and a take charge individual.

 Job Responsibilities: Provides education about diabetes and self-management to patients with diabetes, their families, and/or caregivers and support systems, provides education about the diabetes disease process, nutrition, exercise, medications, blood glucose monitoring, complications and their prevention, goal-setting, psychosocial adjustment, and other related topics, instructs patients and others in a culturally competent manner, incorporating cultural preferences, beliefs and learning styles, assesses a patient’s educational needs, implements an educational plan for that patient, documents the process in the medical record in a timely, accurate manner, evaluates the outcome of the education based on outcome criteria and adjusts the educational plan as necessary, works closely with other members of the patient care team, including physicians, other providers, nursing staff, the nutritionist, and other essential staff members, develops and conducts group teaching sessions, develops and conducts teaching sessions for the public as needed, participates in and provides expertise regarding clinic projects, such as the Diabetes Registry, the Diabetes Quality Improvement Team, and other future projects.


Qualifications:

  1. Certified as a Certified Diabetes Educator by the National Certification Board for Diabetes Educators.
  2. Maintains an active license as a registered nurse, occupational therapist, optometrist, pharmacist, physical therapist, physician (M.D. or D.O.), physician assistant, podiatrist, or registration as a dietitian by the Commission on Dietetic Registration OR
  3. Holds a minimum of a master's degree, from an accredited college or university in one of the following areas: nutrition, social work, clinical psychology, exercise physiology, health education, or specified areas of study in public health.
  4. Working knowledge of the social and cultural factors associated with the community’s use of social and health care services.
  5. Ability to understand patient attitudes and needs, develop and orient program goals and concepts accordingly, and to work effectively toward their accomplishment with all staff members.
  6. Ability to establish and maintain effective working relationships with coworkers, officials, the public, patients, other private and public organizational representatives, health care personnel and community groups and organizations.
  7. Ability to work under pressure, maintain personal composure, evaluate situations and act/react appropriately, and use tact in dealing with visitors, patients and difficult situations.
  8. Ability to identify and maintain confidential material.
  9. Ability to interpret and conform program operations to applicable laws, rules, regulations and funding source requirements and according to acceptable standards.
  10. Spanish speaking preferred.
