Job Description
Join our team who is committed to the delivery of the highest quality health care service.  We are seeking a full-time HIV Medical Case Manager that is self-motivated, energetic, and a take charge individual. Medical case management is a collaborative process that assesses patient needs and works to develop service plans that facilitates recommended medical treatment plans appropriate medical care is provided.

Job Description:  Under the guidance of the HIV registered nurses, oversee a panel of stable, compliant HIV patients to facilitate required clinic visits, labs and other services as ordered by RN or HIV Physician, conduct annual intakes into the HIV Case Management Program and enroll eligible patients into the Ryan White Program, document all of these patient interactions in Intergy (EMR) and ensure that all patients are up to date with their service and treatment plan, conduct Acuity Scores in compliance with the Wisconsin AIDS / HIV Programs Medical Case Management Guidelines, conduct Annual Ryan White intakes to ensure Ryan White eligibility of your caseload and facilitate their yearly comprehensive case management assessment, facilitate the completion of income verification for all clients every six months, become proficient in SSCHC electronic medical record (Intergy) and Intergy Base to allow for data entry, obtaining clinical data and report generation, develop a process for tracking patients who routinely miss medical appointments or have become lost to care, through use of Excel, interpreting reports from Practice Analytics and communication with the HIV RN case managers and social worker, assist patient in scheduling appointments to return to care and track compliance, facilitate linkage to AODA/Mental Health assessment and treatment for all patients requiring these services, in collaboration with other members of the HIV staff, contact area inpatient and outpatient treatment programs and methadone clinics to determine requirements for entry into program and availability of Spanish-speaking counselors, track compliance of patients enrolled in AODA/Mental Health programs, develop a process for tracking patients who require behavioral health intervention but are not compliant, attend monthly combined meetings with Behavioral Health Department and ID consultant to discuss patients with whom you interact, ensure that all necessary information, documents and evaluations are in the official case record, participate as an active member of the HIV Department including staff meetings, in-service training and workshops, department problem solving exercises and providing support and peer mentoring to coworkers, provide other case management services to HIV clients in the absence of the social worker or when directed by HIV Director, implement special projects and participate in community events as assigned by your supervisor

Qualifications:
  1. Bilingual (Spanish/English) required.
  2. Three years’ experience in case management, medical assisting or medical case management.
  3. Completion of Medical Case Management certification within 1 year of hire date.
  4. Knowledge of medical terminology.
  5. Experience in providing services to individuals with HIV or other chronic diseases preferred.
  6. Must be fully cognizant of the legal and moral issues related to confidentiality around HIV/AIDS.
  7. Effective oral and written communication skills including the ability to network effectively within the healthcare and social welfare systems.
  8. Ability to relate well to people from diverse ethnic and cultural backgrounds.
  9. Valid Wisconsin driver’s license, use of an automobile and current automobile insurance.
