What You Will Do:

The Advanced Practice Provider provides diagnostic and therapeutic health care to patients through assessment, diagnosis, and treatment in the ambulatory care setting in collaboration with a designated PHMG physician. 

Responsibilities
• The Nurse Practitioner is responsible for assessment, diagnosis and treatment of patients aged birth to geriatric. Conditions managed include acute illnesses in the ambulatory care setting. Refers for more urgent care when warranted.
 
• Obtains medical histories and ensures relevant documentation in accordance with PHMG standards, the state of Wisconsin, PHMG Scope of Practice and collaborative agreement with PHMG physician.
 
• Interprets and evaluates patient care data in order to determine management and treatment of diagnoses in accordance with best practice, current relevant clinical guidelines and medical processes. Orders and interprets diagnostic studies in the field of trained clinical specialty. Performs minor and routine procedures.
 
• Encourages patient involvement in care through counseling, instructing and education. Promotes active discussion with patient regarding plan of care, treatment options, care outcomes, preventative health measures and self-care skills.
 
• Provides coordination of referral services including managing a variety of ancillary services.
 
• Prescribes, recommends and regulates both pharmacologic and non-pharmacologic therapies including controlled substances, as warranted for patient specific conditions.
 
• Consults with physician as appropriate.
 
• Documents patient encounters, assessments and communication in timely manner.
 
• Provides clinical direction/delegation to nursing staff regarding interventions identified in the plan of care. Follows through with nursing staff to ensure the patient receives recommended care.
 
• Takes an active role in identifying and creating opportunities to educate, motivate and collaborate with staff and students in their professional development.
 
• Participate in APP governance with adherence to standard pillars of practice as defined by individual governance structures. Engage in quality related process improvements.
