We are seeking an Thoracic Transplant Clinical Pharmacist to:

• Be accountable for working directly with multidisciplinary teams in the transplant clinic to provide high-quality evidence-based care.
• Work with a growing thoracic transplant program with increasing heart and lung transplant volume and providers we are looking for an individual to establish our practice in these patient populations. With strong existing practice in abdominal transplant and cardiology.
• Take the opportunity to build on previous success while charting a new course alongside strong provider support.
• Work primarily in the clinic position but, also staff on our adult inpatient units. Part of this position would encompass inpatient staffing, holiday, and weekend requirements.

Education:

Minimum - Bachelor's degree in Pharmacy or a PharmD degree.

Preferred - Completion of ASHP Accredited PGY1 Practice/Community and PGY2 Transplant Residency training program.

Work Experience:

Minimum - One (1) year of clinical patient care and orders management experience

Preferred - Three years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program

Licenses and Certifications:

Minimum - Licensed to practice pharmacy in the State of Wisconsin or must obtain within 90 days of hire date

Preferred - Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.).

• Comfortable accepting responsibility for medium to large scale projects involving multiple resources and spanning many months from start to finish

Physical Requirements:

Light: Ability to lift up to 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.

Work Schedule:

This is a full-time, 1.0 FTE, salaried position, scheduled between 7:00 am to 9:00 pm, about 40 hours per week, with about 1 in 4 weekends. Hours may vary based on the operational needs of the department.
