Job Description
Join our team who is committed to the delivery of the highest quality health care service.  We are seeking a full-time Maternal & Infant Lead Coordinator that is self-motivated, energetic, and a take charge individual.
 
Summary of Duties:

The Sixteenth Street Community Health Center Lead Outreach Program (LOP) is a dynamic team protecting the health and well-being of youth on Milwaukee’s south side by helping educate families about how to prevent and mitigate the harmful effects of lead poisoning and create a healthy environment in their homes. The Maternal & Infant Lead Coordinator (MILC) plays a key role by educating pregnant women before their baby is born and performing a home assessment to identify potential hazards in the home with a goal of preventing lead poisoning in infants before it starts. The MILC will build relationships with entities within and outside of SSCHC to mobilize a unified effort to help families on the South Side of Milwaukee. Spanish and English fluency is essential.

Job Responsibilities:
  1. In collaboration with the Manager of the Department of Environmental Health & Community Wellness, develop a framework and programming for pre-natal and infant lead poisoning prevention education with the help of LOP, WIC, and Women’s Wellness staff at SSCHC.
  2. Serve as the primary contact for prenatal mothers and the families of infant patients enrolled in Sixteenth Street’s Lead Outreach Program.
  3. Coordinate office visits in the first trimester and in-person and virtual home visits in the third trimester for families to educate about lead poisoning and discuss ways to stay healthy and prevent and decrease the harmful effects of lead on children under one year of age. Follow up with these families at regular intervals to offer support, encouragement, and further education as needed.
  4. Work closely with Sixteenth Street’s WIC and Women’s Wellness departments to recruit expecting mothers into the Lead Outreach Program and follow up with consistent communication throughout the pregnancy and within the first year of life.
  5. Coordinate with other members of the Lead Outreach Program to hand off patients over one year of age to ensure timely follow ups with the family as the children age within the program.
  6. Build relationships and offer educational opportunities with other community organizations that serve mothers and infants on the south side to leverage a network of support. This includes giving developing and giving educational presentations to the public in partnership with the Community Lead Outreach Worker in LOP.
  7. Customer Service: Respond to client's with questions, comments, concerns or complaints with a purpose to a resolution.
  8. Maintain thorough records of consultations, home visits, enrollment, and recruitment efforts and compose reports for our grant funders in collaboration with the Manager. Help to populate monthly, quarterly, and annual reports as needed.
  9. Collaborate with Manager of DEH&CW and internal and external partners on strategies to meet monthly goals.
  10. Help manage client records in internal LOP and SSCHC client databases.
  11. Work closely with colleagues in the Lead Outreach Program to assist with home visits, education efforts, tabling, etc.
  12. Collect blood samples from children 5 and younger to test for elevated lead levels.

Qualifications: 
  1. Must be fluent in Spanish and English.
  2. Must have strong background in customer service and administrative office skills.
  3. Must be able to maintain filing systems and proficient typing speed of 40 wpm minimum.
  4. Must be experienced in MS Office suite.
  5. Attention to detail, highly organized.
  6. Ability to meet driving requirement, including clean driving record.
  7. Familiarity with the communities on the south side of Milwaukee a plus.
