Sixteenth Street Community HealthCorps - Resource NavigatorSixteenth Street Community Health Centers (SSCHC) is a federally qualified community health center based in the Southside of Milwaukee and we have been providing various services and resources to our community for over fifty years. The focus of Sixteenth Street Community HealthCorps is aimed towards enhancing our members professional development, personal fulfillment as well as our agency s capacity to provide quality services and resources to our patients and community in a safe and welcoming environment. Our program s service year begins in September and each member will be providing 1,700 hours of service within their selected department. Service provided by our members reflect the needs of our community and will support Sixteenth Street s potential of improving the health and well being of our patients and local residents.Member Duties : 4 full-time members serving as Resource Navigators will enhance the capacity of our Social Services Department to reach and assist patients with access and navigation of community resources and systems to meet their needs related to Social Determinants of Health (SDoH). Activities include: Apply SDoH screening to patients during Primary Care visits in clinic setting. Assist patients reporting needs to connect them to internal or community resources. Conduct patient satisfaction surveys with patients who received navigation services at our agency. Recruit ongoing and episodic volunteers to support the program. Educate patients about self-advocacy, show them how to research and access resources needed when facing critical life situations. Conduct survey with patients for behavior change or intent to change behavior to improve health.
