Job Description
We are seeing a full-time Asthma Education Specialist.  
 
Job Responsibilities:
  1. Conduct home visits to reinforce family-patient asthma management skills including:
    • Conducting in-home environmental assessments to identify potential asthma triggers.
    • Assisting patients and their family to identify their personal asthma trigger(s)
    • Understand, identify and educate about indoor and outdoor allergens and/or irritants that can make asthma worse and ways to reduce exposure to these triggers.
    • Demonstrate low-cost trigger management strategies.
  2. Administer Spirometry to referred patients of SSCHC:
    • Address referral from provider.
    • Connect with patient to schedule Spirometry.
    • Administer Spirometry to both adult and pediatric patients of SSCHC.
    • Patient education for proper medication use and dosage.
    • Document results and task provider assigned to interpret results.
    • Participate in trainings and maintain up to date on Spirometry best practices.
    • Assist with cross-training CCHED staff when coverage is required.
  3. Participate in asthma education classes and activities.
  4. Assist with any Asthma Quality Improvement Projects as needed.
  5. Responsible for environmental data collection and record-keeping.
  6. Facilitate Medication Assistance Program (Drug Assistance) for under and uninsured patients to better afford their medications:  
    • Assist patients with application process for the appropriate manufacturer/program to determine eligibility.
    • Contact drug manufacturers as necessary throughout application process.
    • Document/track progress of patient application/receipt of medications in EMR and drug assistant software.
    • Monitor medication refrigerator to ensure proper temperature daily; arrange for coverage if absent.
    • Follow proper procedure for labeling and storing medications when received.
    • Assist with training volunteers/staff in the Medication Assistance Program as needed.
  7. Assist CCHED staff as needed for coverage (glucometer teachings, facilitating appointments with CCHED team).
  8. Comply with SSCHC TB Control Plan including PPD testing.
 
Qualifications:
  1. Associate Degree in Health or Environmental related field.
  2. Experience in community-based agency, or community health organization preferred.
  3. Ability to present information to individuals and groups.
  4. Ability to interact with patients and families of various cultural backgrounds.
  5. Bilingual verbal and written communication skills required (Spanish/English).
  6. Valid Wisconsin driver’s license, use of an automobile and current automobile insurance.
  7. Excellent time management skills and the ability to multi-task.
  8. Basic computer skills.
  9. BLS Certification.
