Sixteenth Street Community HealthCorps - Family Education

Sixteenth Street Community Health Centers (SSCHC) is a federally qualified community health center based in the Southside of Milwaukee and we have been providing various services and resources to our community for over fifty years. The focus of Sixteenth Street Community HealthCorps is aimed towards enhancing our members' professional development, personal fulfillment as well as our agency's capacity to provide quality services and resources to our patients and community in a safe and welcoming environment. Our program's service year begins in September and each member will be providing 1,700 hours of service within their selected department. Service provided by our members reflect the needs of our community and will support Sixteenth Street's potential of improving the health and well being of our patients and local residents. Member Duties : 1 full-time Family Education Program AmeriCorps Member will spend about 70% of their time enhancing programing for families by facilitating children education in nutrition, cooking and physical activity as well as coordinate logistics. This member will serve about 30% of their time in the Healthy Schools Program and the Community Outreach Program. Activities include: Expanding recruitment of children. Provide direct linguistic and culturally appropriate health education (nutrition, fitness) to children during the cycles of 12 week family education program and the series of 4 week family cooking classes. Recruit and coordinate volunteers. Track patients BMI and behavioral changes and have data readily available for evaluations. Develop marketing materials and content for social media platforms. Conduct survey with patients for behavior change or intent to change behavior to improve health. Avoid all prohibited activities while serving or representing AmeriCorps.
