Sixteenth Street Community HealthCorps - Women's Wellness

Sixteenth Street Community Health Centers (SSCHC) is a federally qualified community health center based in the Southside of Milwaukee and we have been providing various services and resources to our community for over fifty years. The focus of Sixteenth Street Community HealthCorps is aimed towards enhancing our members' professional development, personal fulfillment as well as our agency's capacity to provide quality services and resources to our patients and community in a safe and welcoming environment. Our program's service year begins in September and each member will be providing 1,700 hours of service within their selected department. Service provided by our members reflect the needs of our community and will support Sixteenth Street's potential of improving the health and well-being of our patients and local residents. Member Duties : 2 full-time Women's Wellness AmeriCorps Members will enhance the educational programing of the Women's Wellness Department by facilitating education for pregnant women and for parents of newborn babies. No abortion services or referrals will be provided. Activities include: Recruitment of members for prenatal, breastfeeding, parenting, car seat, and relaxation classes. Ongoing contact with pregnant women within 2 weeks of referral to answer all questions and make sure patient followed through with previous recommendations. Verify pregnant patients are attending routine appointments and have follow up appointments scheduled at all times. Provide linguistically and culturally appropriate assistance to SSCHC pregnant patients. Keep patient education material updated and organized. Conduct survey with patients for behavior change or intent to change behavior to improve health. Member will avoid all prohibited activities while serving or representing AmeriCorps.
