About the Role: Summary: This position is responsible for coordinating the referral and prior authorization processes for assigned Patient Centered Medical Home (PCMH) teams and their respective patients and assisting uninsured clients in gaining access to specialty medical services. The Referral Specialist consistently provides the highest level of customer service over the telephone or in person. Location: 2303 Dodge Ave, Waukegan, IL Hours: 37.5 hours a week; Monday-Friday (8:00am-4:00pm) FLSA STATUS: Hourly Referral Specialist Skills and Experience Coordinates all referral requests and follows up on necessary documentation. Assists in ensuring that referrals are addressed in a timely manner. Assists PCMH team in obtaining prior authorizations as required by the patients insurance company. Coordinates obtaining results from all referrals. Documents and informs patient of referral, prepares any required forms and transmits required information to outside providers and/or insurance company. Determines patients eligibility for the uninsured specialty referral grant payment program. Schedules patients for appointments, follow-up appointments, tracks results and recommendations. Establishes and maintains cooperative relationships with Lake County Health Department and Community Health Center (LCHD/CHC) staff, insurance companies, local hospitals, physicians and other health care providers to facilitate continuity of care for LCHD/CHCs patients. Serves as a point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns and assists in solving potential issues related to the healthcare system(e.g., request interpreters as appropriate, transportation services or prescription assistance)Prepares correspondence and any other required documentation related to above position responsibilities. Accurately performs data entry of program related data and information in the appropriate computer systems. Job Skill Assessment(s) may be required for this role. Referral Specialist Education, Qualifications, and Certification: High School diploma or GED required, some college preferred. Previous experience in a medical office with thorough understanding of Medicaid, Medicare, Managed Care Organizations and PPOs preferred Data entry and basic computer skills required.
