Job Description
Join our team who is committed to the delivery of the highest quality health care service.  We are seeking a full-time Licensed Practical Nurse that is self-motivated, energetic, and a take charge individual.

Responsibilities:
  1. Render excellent customer service and enhance the patient/clinical team relationship:
  • Greet warmly and supportively.
  • Introduce self and inform on next steps.
  • Inform patient about any pertinent time issues.
  • Explain procedures, proactively seek inform/clarify.
  • Express thankfulness, offer service and advocacy, give choices.
  1. Assist in smooth flow of clinic, including but not limited to:
  •     Greet and establish clinical relationship.
  • Prepare for clinic by reviewing charts for patient needs.
  • Providing translation for non-bilingual staff.
  • Set-up and stocking of exam room.
  • Prepare patients for medical visits by taking histories, BP, HT, WT, temperature, Respiratory Rate, Pulse, and anticipatory screenings based on the chief complaint, as needed (vision and hearing screening, EKG, NST, CLIA testing).
  • Prepare anticipatory referrals.  
  • Manage flow of exam rooms.
  • Work to maintain a manageable schedule by noting openings and anticipating complex patients who may require additional MA support.
  1. Perform CLIA testing, to include but not limited to:
  • Routine CLIA lab procedures as qualified.
  • Collect specimens and prepare for analysis.
  • Complete related documentation.
  • Alert provider of abnormal results as per policy.
  • Maintain cleanliness of lab area, restock lab as necessary.
  1. Assist in maintenance of EMR patient documentation:
  • Review patient data on chart each visit.
  • Assist nurse with follow-up or project activities as assigned.
  1. Assist patient with exiting material:
      -   Reinforce plan of care and new medications directions.
      -   Assist with referrals as needed.
      -   Render support towards avoiding additional stops or hand-offs.
  1. Provide patient centered nursing care in an outpatient clinic according to established scope of practice guidelines and standards of care.
  2. Administer medications, under physician/provider orders, including immunizations, antibiotics, pain medications, vitamins, topical agents, Synagis, and insulin. Initiates, maintains, and discontinues IV therapy: administering selected fluids.
  3. Assist the triage nurse with screening calls.
  4. Provide basic patient education on the performance of tasks, skills, treatment, and self-care.
  5. Refers questions to registered nurses and physicians/providers per scope of practice guidelines and relays information back to patient as directed.
  6. Participate and maintain projects as assigned. May include but not limited to:
  • Become CPR instructor certified and teach SSCHC classes.
  • Assist RNs with assigned projects.
  • Assist Medical Assistant Supervisor with compliance and safety procedures.
Qualifications:
  1. Graduate of accredited school for Licensed Practical Nursing
  2. Licensed Practical Nurse with current license
  3. Current basic life support (BLS) certification or certification within 6 months through in-house training
  4. Ability to relate well to people from diverse ethnic and cultural backgrounds.
  5. Bilingual (Spanish/English) preferred.
