Heritage Senior Living is currently seeking passionate Licensed Practical Nurses (LPN)/Clinical Supervisors to join our senior living community and provide quality care to our residents! We are currently seeking candidates for out PM shift (afternoon-evening). Our plan is to open one to two new senior communities per year and develop centers of excellence that provide comfort, care, quality, and specialized dementia services.The clinical supervisor/LPN's primary role is to evaluate, plan, and deliver nursing care in collaboration with the DON, in accordance with company policies and DHS 83 and 89 state regulations.Essential Duties and ResponsibilitiesPossesses knowledge of nursing principles (geriatrics, senior living, and long-term care), professional standards of nursing practice and ability to apply to resident specific circumstancesAssume self-responsibility for all direct patient care activities as indicated by the DON within the scope of the Nurse Practice Act, DHS 83 and with HSL policies and proceduresPossesses knowledge of Wisconsin state regulations for CBRFs and/or RCACsProvide direct patient care according to the ISP and physician ordersProvide appropriate information to assist the DON in the preparation and maintenance of an effective ISPDocument detailed notes in residents' electronic medical records that reflect the therapeutic plan and specific aspects of the ISPAdminister prescribed medications to residents in accordance with physicians' ordersCarry out duties in accordance with the facility's mission, vision, and core valuesDemonstrate honesty and integrity in the care and use of resident and facility propertyMaintain a good rapport with familiesReport and investigate reportable or sentinel events for their root cause, including reviewing progress notes and incident reports and developing a plan to minimize the reoccurrence of the eventComplete follow-ups on:MD notification or change of conditionFamily notification of change of condition, labs, physician orders, etc.Accidents/incidentsSkin alterationsNew admission chart review and clarificationsDischarge chart reviewAfter follow-ups, update ISPs as warranted and perform physical evaluations related to change of conditionOversee the 24-hour report sheet. Make routine checks on any resident who is acutely ill or has any change in conditionCommunicate with all departments to ensure quality care of residentsMaintain effective, positive working relationshipsOversee the care provided by caregivers/CNAs, as well as ensuring their documentation is accurate and complete. Counsel and discipline caregivers/CNAs as necessaryMaintain high professional standards and represent the facility wellAttend and participate in weekly leadership meetingsProvide education to care staff as necessary and requiredPerform other duties as assigned by the director of nursingParticipate in daily care focus meetingParticipate in weekly care coordination meetingEducation/Qualifications:Valid State of Wisconsin Nursing LicenseLicensed Practical Nurse/Licensed Vocational NursePossess knowledge of nursing principles (geriatrics, senior living, and long-term care) and professional standards of nursing practice and have the ability to apply them to resident specific circumstancesPossess knowledge of Wisconsin state regulations for CBRFs and/or RCACsPossess sufficient organizational and interpersonal skills to assign tasks, encourage staff, produce positive outcomes, and improve qualityMust be in good standing on the State Board of Regulation and LicensingAbility to speak, write, and read EnglishMust be self-directed and able to function independentlyMust be flexible and able to handle multiple variations in a daily scheduleAbility to communicate well written and verballyAbility to travel as neededPossess legible handwritingMust submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal regulations Competency and ability to utilize a floor lift, sit-to-stand and friction-reducing devicesMust have basic computer knowledge, including EHR software systems Essential FunctionsTo perform this job successfully, an individual must be able to perform each essential duty.
