Women’s Wellness AmeriCorps Member

Member Position – Summary and Purpose

Two (2) full-time Women’s Wellness AmeriCorps Members will enhance the educational programing of the Women’s Wellness Department by facilitating education for pregnant women and for parents of newborn babies. No abortion services or referrals will be provided.

Member Duties and Responsibilities
  • Sustain enrollment of SSCHC pregnant patients in SSCHC programs that the patient qualifies for.?
  • Recruitment of Perinatal Care Coordination members for prenatal, breastfeeding, parenting, car seat, and relaxation classes?
  • Ongoing contact with pregnant women within 2 weeks of referral to answer all questions and make sure patients followed through with previous recommendations
  • Complete the annual United Way Surveys
  • Verify pregnant patients?are attending routine appointments and have follow up appointments scheduled at all times
  • Verify whether pregnant patients are enrolled in state insurance
  • Verify that all pregnant patients have had initial intakes scheduled with a Perinatal Case Manager
    ?
  • Increase use of health services and programs by providing linguistically and culturally appropriate assistance to SSCHC pregnant patients.? 
  • Should it be determined that further follow up from a previous referral (Dental, Chronic Care, WIC, Social Services, AODA, Medical-Educational Specialist etc.) is warranted, member will facilitate the delivery of needed services to the patient.
  • Offer appointment with chronic care specialist for all pregnant patients who have been referred by the Perinatal Case Manager
  • Inform all pregnant patients about SSCHC’s bilingual prenatal and breastfeeding classes and encourage them to attend
  • Keep patient education material updated and organized. Make copies needed for all programs that are designed for the pregnant patients
     
  • Provide Health Education:?
  • Member may design a class or develop health topic to be discussed at individual appointments?? 
  • Provide one-on-one health education to pregnant women
  • Provide nutrition education to pregnant women which includes discussing healthy weight gain, diet, water consumption, etc. in pregnancy based on BMI chart
  • May provide health education to families within the community through SSCHC Healthy Choices program
  • Donation Drive?
  • Coordination of donation drive (donations?include?maternity and baby clothes for Womens Wellness Department)
  • Organize a community baby shower?? 
  • Facilitate and offer our new?program to offer safe sleep and baby items for PNCC moms 
  • Promotion of donation events through social media and community outreach??
  • Overall
  • Member will have the opportunity for mentorship, collaboration projects and event planning across Sixteenth Street Community Health Centers.
  •  
  • Collect and report data of individuals served on a change of behavior or intent to change behavior for a healthier lifestyle
  • Member will avoid all prohibited activities while serving or representing AmeriCorps.
Qualifications and Experience
  • Be a high school graduate, GED recipient, working toward attaining a high school diploma or GED during the term of service. Members must agree to obtain either a diploma or GED before using an education award.
  • 18 years old or older, no upper age limit
  • Hold one of the following citizenship statuses: US citizen, US National, Lawful Permanent Resident Aliens
  • Pass a National Service Criminal History Check
  • Studies and/or experience in Public Health, Health Sciences, Pediatric and Child Development Social Work, Human Services, Case Management, or related areas.
  • Experience with health education (pregnancy and children), have commitment to serve communities of color, and true desire to help adults and children to live healthier lives.
  • Bilingual English/Spanish is preferred.
     
Line of Supervision

The Women’s Wellness AmeriCorps Members will report directly to the Women’s Wellness Team Leader.
