Job Description

To provide comprehensive Registered Nurse Case Management and nursing care to maintain/improve the quality of life for individuals infected with HIV/AIDS. Coordinate comprehensive healthcare. Collaborate with other healthcare providers and community agencies. Independently develop, implement, coordinate and evaluate healthcare education in an ambulatory setting.

 

POSITION-SPECIFIC COMPETENCIES/ESSENTIAL FUNCTIONS/DUTIES & RESPONSIBILITIES

 
  • Facilitate client access to healthcare and community resources. Assist client to develop natural resources and make appropriate referrals related to barriers to healthcare (i.e. housing, transportation).
  • Provide nursing care within the scope of practice, performs clinical duties, and assist physician during clinic sessions.
  • Maintain clinic cleanliness and order (i.e. stocking rooms, rotating medication, etc.).
  • Document all encounters and contacts with clients.
  • Maintain and update patient data in electronic medical records.
  • Collect and organize data and reports as appropriate for all grant reporting.
  • Participate in departmental and organizational meetings and programs, including but not limited to community outreach and health fairs.
  • Seek opportunities to increase clinical, professional and personal growth independently and through in-house education, conferences and community resources.
  • Provide in-house and community presentations related to HIV/AIDS education.
  • Work collaboratively, constructively, and cooperatively with team members.
  • Perform other job duties as may be assigned.
Work independently with clients to assess needs related medical, social and behavioral health services.

 

POSITION REQUIREMENTS

 
  • Experience: One year of general nursing experience in clinic or hospital setting. HIV experience preferred.
  • Education Graduate of an accredited registered nursing program. Must be currently licensed in the State of Wisconsin
  • Expertise:   Excellent clinical, critical thinking, and interpersonal skills are a must. Strong oral and written communication skills are required. Effective organizational skills and computer literacy is critical. Ability to be self governed and to work with diverse constituencies within the community including heath care professionals, community-based organizations, and governmental agencies. Must be willing to travel locally and out of state. A valid driver’s license and reliable insured vehicle is required.
