Job Description

Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Social Service Intake that is self-motivated, energetic, and a take charge individual.

Responsibilities:

  1. Complete intake assessment with all patients presenting to Social Services.
  2. Assist and advocate for patients presenting with base line level psychosocial needs by connecting them with providers and internal and external resources.
  3. Schedule appointments for patients with a Patient Navigator or Social Worker when presenting with moderate or complex needs.
  4. Provide reminder calls to all patients scheduled in Social Services.
  5. Educate patients to self-advocate and to learn about resources needed for when facing critical life situations.
  6. Monitor and respond to tasks from medical team within the Electronic Health Record (EHR).
  7. Document thoroughly on all patient interactions in the EHR.
  8. Participate as an active member of the Social Service Department including staff meetings, in-service training and workshops, department problem solving exercises and providing support to co-workers.


Qualifications:
1. High school diploma or GED.
2. Excellent oral and written communication skills.
3. Working knowledge of computers and office applications software.
4. Strong organizational and problem-solving skills.
5. Ability to work independently and within a team setting, navigate multiple competing priorities, and be a self-starter.
6. Experience working with individuals from diverse ethnic and cultural backgrounds.
7. Bilingual written and oral proficiency (Spanish/English) required.
